All of the following forms are required for Athletic and Activity participation
at Sam Barlow High School. One additional form is NOT available for download,
it is the parent release, insurance information and travel release form and
is available
from your coach or the Athletic Office. All forms are pdf format.
Physical Form
This form is required by the OSAA and is the only form that can be accepted.
All freshmen, Juniors and students new to Barlow are required to have a physical.
Physicals are good for two years. Freshmen physicals have to be dated January
1 of the year they enter Barlow or later.
